All patients are asked to complete a comprehensive medical history form detailing your injury or condition, with questions about how the injury occurred and how it effects your day to day life.

We can email this form out to you for completion at home or you can complete it in our rooms prior to your appointment.   If you would like to compelte the forms in our rooms we will ask you to come in 30 Minutes prior to your appointment time to ensure the forms are completed prior to your appointment.

If you have any reports from Xrays or Scans relating to your injury or condition please bring them in.   If you do not have copies please let our staff know where you had the Xray or scan done and we can try and obtain copies of the reports.

If you are driving in, you may be with us for up to 90 minutes so please ensure you park in a 2 hour car park.  We do front onto a multistorey car park but fees do apply, otherwise there are a number of 2 hour car parks on the surrounding streets. 

Due to Covid restrictions only patients, support persons or carers who are attending the appointment can stay in the waiting rooms.  

Masks must be worn within the Practice if you have Covid, flu like or cold symptoms .
